Prograf Dosage

Your age, weight, and other medications you are taking are just a few of the factors your healthcare provider will consider when determining your dose of Prograf. After your transplant surgery, you may be started on the injectable form of this drug, as you may have problems swallowing Prograf capsules. However, most people will eventually take the capsule form of Prograf to prevent organ rejection.

An Introduction to Dosing With Prograf

The dose of Prograf® (tacrolimus) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • The reason you are taking the medicine
  • How you respond to it
  • Your age
  • Your weight
  • Your Prograf blood levels
  • Other medical conditions you may have
  • Other medications you are taking.
 
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.
 

Dosing Guidelines for Prograf Capsules

Prograf comes in an oral form (capsules) and an injectable (IV) form. Although some people will be started on the IV form, most people will eventually be put on the capsule form.
 
Dosage for Preventing Kidney Transplant Rejection
The recommended initial Prograf dosage after a kidney transplant is 0.05 mg per kg twice a day (about 0.023 mg per pound) when used in combination with azathioprine (Imuran®) and 0.1 mg per kg twice a day (about 0.045 mg per pound) when used in combination with mycophenolate (CellCept®, Myfortic®). The first dose is given as soon as the kidneys appear to be functioning after the transplant surgery.
 
People with severe liver disease may require lower Prograf doses.
