Neoral Dosage for Rheumatoid Arthritis

The initial dose of Neoral for the treatment of rheumatoid arthritis is usually 2.5 mg per kg of body weight each day (1.14 mg per pound), divided into twice-daily dosing. It normally takes four to eight weeks for the medicine to start working to relieve symptoms.
 
If you do not notice an improvement by eight weeks and you are tolerating the medicine well, your healthcare provider may increase your dosage by 0.5 mg to 0.75 mg per kg per day (0.23 mg to 0.34 mg per pound). This amount may be increased, if needed, after 12 weeks of treatment. The maximum recommended Neoral dose is 4 mg per kg per day (1.8 mg per pound).
 
If your arthritis symptoms are still not relieved within 16 weeks, it is generally recommended that treatment be stopped. Your healthcare provider may also recommend you stop taking Neoral, or reduce your dosage, if you experience potentially serious side effects at any time during treatment.
 

Neoral Dosage for Psoriasis

The initial recommended dosage of Neoral for the treatment of psoriasis is typically 2.5 mg per kg body weight each day (1.14 mg per pound), divided into twice-daily dosing. If your psoriasis is not adequately controlled after four weeks of treatment, your healthcare provider may begin to increase your dosage amount, usually by about 0.5 mg per kg per day (0.23 mg per pound).
 
Further dose increases may be made every two weeks, until an adequate response is observed, serious or bothersome side effects occur, or you reach the maximum recommended dosage of 4 mg per kg per day (1.8 mg per pound).
 
You may notice some improvement in your condition within two weeks of starting treatment; however, it may take 12 to 16 weeks for a complete response. It is generally recommended that treatment be stopped if an adequate response is not achieved after six weeks of treatment at a dosage of 4 mg per kg per day (or the highest tolerated dose).
 
Once you have achieved an adequate response, your healthcare provider may attempt to lower the amount to find the lowest effective dose for you. Some people may have adequate relief on doses less than the starting dosage (2.5 mg per kg per day).
